What to Bring to Your First Appointment
You will be asked to fill out several new patient forms prior to meeting the doctor. These forms can be downloaded above, completed and brought with you, or you can fill them out in the patient waiting room. Please also bring:
- Health Insurance Information
- Picture Identification
- Proof of Address
Please arrive 15 minutes early to complete your paperwork if you have not already done so.

Insurance and Fees
We make every effort to keep down the cost of your medical care without sacrificing quality. To help us meet this goal we ask that you pay the co-payment and/or co-insurance due at the time of your visit. We will be happy to file the primary and secondary insurance claims as a service to you. Please remember that you are responsible for any deductible, co-insurance, or other balance not covered by your insurance company.
Insurance policies can be very complex. Please understand that health insurance is a contract between the patient and the insurance company, not between the insurance company and the physician. Payment of the account is ultimately the patient's responsibility. Our Insurance Department is available to assist you.
At the time of your visit a staff member will discuss fees with you. Financing options are available.
Our office charges a nominal fee for completing disability and worker's compensation forms.
