Do I need a doctor’s referral to make an appointment with you?

We see patients by appointment as requested by other physicians. You can schedule your own appointments; however, you should have a primary care physician for your routine medical needs and treatment.

What do I need to bring to my appointment?

If you’re a new patient, please bring your completed new patient packet. Also, please bring your insurance information, a photo ID and proof of address. If you have had previous tests or studies relating to your condition, please have your doctor send this information to us. Please arrive 15 minutes early to complete your paperwork if you have not already done so.

Do you accept insurance?

We work with many insurance companies and will be happy to file your primary and secondary insurance claims as a service to you. We ask that you pay the co-payment or co-insurance at the time of your visit.

What if my insurance will not cover a particular procedure?

At the time of your visit a staff member will discuss fees with you. Financing options are available.

Can you complete disability and/or workman’s compensation forms?

Our office charges a nominal fee for completing disability and worker’s compensation forms.

What are your office hours?

Office hours are Monday through Friday 9:00 am to 5:00 pm. Appointments are scheduled on Tuesday from 9:00 am to 4:00 pm and on Wednesdays and Fridays from 9:00 am to noon.
